Heartland, already the largest DSO in the US, added one practice in Nevada in February and five more offices in April. Heartland is now consistently the most active acquirer we see month-over-month. Bridge picked up Foothill Family Dental Group and Peak Dental Specialists in Glendora, California, its third and fourth practices in California and its 27th–28th nationwide. The interesting signal: Bridge is now visible in regional consolidation alongside the bigger players. They are not on most owners' radar yet, that's exactly when sellers tend to get less competitive offers.
③ Bridging the Atlantic: Dental Care Ireland keeps adding
Dental Care Ireland is the most active acquirer in our database that operates entirely outside the US, five tracked deals over the past three years, with the most recent in December 2025. For European operators (and US groups eyeing Irish/UK entry), Dental Care Ireland's expansion pattern is a useful read on cross-border consolidation pace.
④ MB2 Dental's partnership model continues compounding
In November 2025, MB2 announced "a new doctor partner and three new practices across" multiple states. Their model, minority-equity partnership rather than full buyout, keeps outperforming pure roll-ups on retention of legacy clinical leadership. ⑤ Specialty roll-ups: EPIC4 Specialty Partners acquires Bowers Orthodontic

This "fortress region" approach is becoming the dominant playbook for sub-50-office DSOs that can't compete on national scale but can dominate regionally.
